Physical Therapist (PT) – Per Diem | $100–$120/Visit | Contra Costa County

Looking for flexible home health work with more manageable travel?

Qualicare Home Health is seeking a Per Diem Physical Therapist to serve patients throughout Contra Costa County with geographically clustered visits designed to improve efficiency and reduce unnecessary driving.

We prioritize assigning patients within a primary territory whenever possible to help support consistency and work-life balance.

Compensation & Perks

Responsibilities

Perform patient evaluations and skilled follow-up visits
Develop and implement individualized care plans
Provide therapy interventions in the home setting
Educate patients and caregivers
Collaborate with interdisciplinary care teams
Complete timely and accurate documentation

Qualifications

Active California PT license
Home Health experience preferred for Per Diem
Strong clinicians interested in transitioning into Home Health are welcome to apply
Strong communication and organizational skills
