You'll have the opportunity to build meaningful relationships with residents while using your clinical skills to promote their health, safety and quality of life. LPN Responsibilities Provide direct nursing care according to resident care plans and physician orders. Administer medications and treatments accurately and safely. Monitor resident conditions and report changes to the appropriate nursing personnel. Assist physicians and other healthcare providers with treatments and procedures. Educate residents and families regarding healthcare instructions and care plans. Participate in interdisciplinary care planning. Assist with resident admissions, transfers and discharges. Maintain accurate and timely documentation. Promote a safe, clean and comfortable environment for residents. Assume charge nurse responsibilities as assigned. Collaborate with the nursing team to provide quality, person-centered care. Qualifications Active Pennsylvania Licensed Practical Nurse (LPN) license. Strong clinical and documentation skills.
