Continuous Performance Improvement/ Integrated Treatment Model Coordinator

CPI Coordinator

The Jewish Board's Children's Community Residences are transforming to enhanced Children's Community Residences that will provide vision-driven, compassionate, high quality, evidence-based services and skills development to youth and families in the communities we serve. The Jewish Board operates 4 Children's Community Residences, 1 in Staten Island and 3 in the Bronx.

Program staff will use a culturally competent, person centered approach to help youth and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. They provide treatment, services and support that prevent hospitalization, reduce the length of time in out-of-home treatment, or re-hospitalization and connect them to needed community-based services. Our programs work closely with community partners to address the holistic needs of youth and families, while also celebrating the strengths and resilience of families and communities.

The CPI Coordinator will be responsible for overseeing the quality of care at 3 Bronx CCRs and SICCR, in addition, he/she will be The ITM Coordinator for all four sites.

Key Essential Functions:

  • Monitor and assist with implementing on-going CPI activities including a systematic quality improvement plan which identifies program strengths and weaknesses.
  • Function as liaison with the compliance department at JBFCS Headquarters as well as other regulatory bodies including Justice Center, OMH, EBA etc.
  • Continually evaluate organizational needs including periodic review of Program Policies and Procedures
  • Oversee and coordinate program efforts in regard to meeting regulatory mandates including the Playbook and utilizing Tableau reports to monitor program compliance.
  • Oversee and coordinate efforts in preparation of audits and accreditation process; this includes follow-up for any correction action that needs to occur.
  • Help ensure quality of case records.
  • Track and oversee proper documentation of events and incidents.
  • Lead monthly CPI meeting and necessary follow-up.
  • Conduct periodic resident satisfaction surveys.
  • Track and monitor training attendance.
  • Oversee and coordinate collection of materials as related to investigations in the program, this includes reviewing camera footage, incident reports, scheduling interviews etc.
  • Complete camera reviews for the programs.
  • Complete UR (Utilization Reviews) for all youth admitted into the program and every six months thereafter.
  • Conduct ITM Observations of all four sites.
  • Complete data collection tasks for ITM (diary cards, BCA's, EBP's and ITM Tokens), as well as OMH data from CAIRS, MyAvatar and other trackers.
  • Complete, in collaboration with program leadership the PIR, OMH Annual survey, and all other needed regulatory submissions.
  • Other tasks as assigned.

Additional tasks:

  • Lead monthly CPI meeting and necessary follow-up.
  • Conduct periodic resident satisfaction surveys.
  • Track and monitor training attendance.
  • Oversee and coordinate collection of materials as related to investigations in the program, this includes reviewing camera footage, incident reports, scheduling interviews etc.
  • Other tasks as assigned.

Team Process :

  • Participate in weekly administrative meetings.
  • Participate in regular staff meetings and training. May be asked to travel to offsite meetings and trainings.

Documentation :

  • Coordinate and collaborate with the administrative team.
  • Reviews incident reports as appropriate following an incident.
  • Compiles event reviews monthly for the program.
  • Complete all trackers needed for the PIR, and OMH data collection.
  • Submit bi-monthly assessments of compliance with regulatory tasks in MyAvatar (Monthly notes, billable services, admission notes, SPRs, URs etc.

Core Competencies :

  • Willingness to work with children/adolescents living with the impact of trauma and experience mental health challenges.
  • Compassion and respect for vulnerable individuals.
  • Patience and understanding for the sometimes-slow process of stabilization and recovery.
  • Excellent youth engagement skills
  • Strong verbal and written communication skills
  • Attention to detail.

Educational/ Training Requirements:

  • BA minimum: Master's degree preferred (or working towards).
  • Experience with Residential Care preferably with Adolescents.

Experience Required/Language Preference:

  • Bilingual, Spanish, French, Creole preferred.

Computer Skills Requirements:

  • Working knowledge of Microsoft Office, Outlook, and Word
  • Connections experience preferred
  • Aptitude for learning new systems and application software.

Visual and Manual Dexterity:

  • The candidate should be able to read paper and electronic documents and perform data entry in various computer programs.

Work Environment/Physical Efforts:

  • Ability to navigate stairs.
  • Ability to respond to crisis, as needed.
