Nurse Care Coordinator — Inpatient Care & Discharge Lead

UW Medical Center - Northwest is seeking a Nurse Care Coordinator to join the Care Management team. The role focuses on achieving coordinated patient flow through inpatient stays and ensuring safe, timely discharges. You will lead interdisciplinary team huddles, address barriers to discharge, and support equipment ordering and care transitions within the UWMC Northwest system.
