Brief Overview Accountable for supporting patient care navigation in the clinic setting. The RN’s primary responsibility is to manage care for the patients of the primary care practice to promote effective education, self‑management support, and timely health care delivery. This includes developing and monitoring care management processes, supporting primary clinical teams, identifying high‑acuity patients, and ensuring care coordination. The position involves some patient triage and will work with clinic leadership and the medical director to develop the role for the best service to patient panels and primary care teams. What you will do Serve as a resource on care management for all patients of the practice, educating patients about self‑management tasks to gain greater control of their health status. Manage patient care in the health care continuum to achieve optimum outcomes in a safe and cost‑effective manner. Collaborate with physicians, providers, and practice staff to identify appropriate patients for care management and provide leadership for patient care teams. Work with social services and nursing to identify, plan for, and provide an appropriate follow‑up plan of care for patients. Communicate the plan of care to patients’ families, medical providers, hospital staff, community support, and other agencies and departments as appropriate. Provide follow‑up contact with patients to ensure compliance with recommendations—medications, labs/x‑rays, specialist visits, PCP visits, dieticians, CDE, etc. Review monthly status reports, dashboard results, and other clinical care delivery information. Communicate with physician groups regarding the statistical and financial impact of care delivery. Serve as a resource for clinic care team education on changing payer requirements. Collaborate with payer case managers for additional services when appropriate. Participate in regular team meetings and peer review activities. Anticipate the needs of this patient population, ensuring necessary documentation and pre‑visit planning is completed before patient visits. Promote patient self‑management and empower patients/families to achieve maximum levels of wellness and independence. Assess barriers when a patient has not met treatment goals, is not following a treatment plan, or has missed appointments. Determine and complete appropriate referrals. Oversee the development, procurement, and adoption of patient self‑management educational resources used by the primary clinical teams. Required Education, License/Certification, or Work Experience Registered Nurse (RN) – Board of Nursing An active license in the state of practice Preferred Education, License/Certification, or Work Experience 1–3 years of clinic nursing experience 4–6 years of case management and/or homecare experience Expectations and Standards Commit to the daily application of Avera’s mission, vision, core values, and social principles to serve patients, their families, and the community.
