The ideal candidate will provide high-quality nursing care to patients in hospice, ensuring comfort and dignity during end-of-life care. This role requires strong clinical skills, empathy, and the ability to work collaboratively with interdisciplinary teams. This position requires on-call responsibilities also./ph3Key Responsibilities/h3ulliProvide skilled nursing care to hospice patients in accordance with established care plans and physician orders./liliMonitor patient condition and report changes to the interdisciplinary team promptly./liliAdminister medications and treatments as prescribed, ensuring patient safety and comfort./liliAssist with pain management and symptom control to enhance patient quality of life./liliEducate patients and families on disease progression, care techniques, and available resources./liliDocument nursing care and patient responses accurately and timely in compliance with regulatory standards./liliCollaborate with physicians, social workers, chaplains, and other team members to develop and update individualized care plans./liliProvide emotional support to patients and their families during the hospice journey./li/ulh3Qualifications/h3ulliCurrent Licensed Practical Nurse (LPN) license in the state of Wisconsin./liliPrevious experience in hospice or palliative care preferred./liliStrong knowledge of hospice nursing principles and end-of-life care./liliExcellent communication and interpersonal skills./liliAbility to work independently and as part of a multidisciplinary team./liliCompassionate, patient-centered approach to care./liliBasic computer skills for documentation and communication./li/ulh3Work Environment/h3pThis position involves providing care in patients homes, nursing facilities, or hospice centers within the New Berlin area.
