Primary Care Physician –Waterbury, CT

COMPENSATION: Salary $250k plus Incentive Bonus’

\n

Our Primary Care Physician provides equitable and effective value-based healthcare to local Medicare patient populations at our innovative network of neighborhood primary care centers. Our Primary Care Physicians are supported by large care teams so our providers can focus on delivering a better quality of care, rather than a volume of services. Role:

\n

\n
• Compensation: Base $230K-$250K (wider range available depending on experience and location)
\n
• Bonus: Based on quality metrics
\n
• Schedule: Monday-Friday 8am-5pm
\n
• Patient Volume: 13-16 a day
\n
• Visits: 20 min. regular visit/ 40 min. full diagnostic and new patient
\n
• Physicians care team: Scribe, MA, Social Worker
\n
• Equipment at clinic: Labs on site with a phlebotomist/ access to RubiconMD (gives access to specialist)
\n
• EMR: Canopy and Greenway
\n
• Model: Value Based Health
\n
• Transportation for patients: Van pick up with in 5 miles of clinic
\n
• Community room patient activities: Exercise, events, yoga, dance, spa days
\n
• Providers per clinic: Average 3-5 (mix of NP’s and Primary Care Physicians)
\n

Required Qualifications:

\n

\n
• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
\n
• Internal Medicine or Family Medicine Board Certification (Or board eligible)
\n
• Active, non-probationary, unrestricted State License
\n
• Bilingual: Spanish Speaking
\n
• No Residents
\n

\n

\n
• Fellowship training in Geriatrics (preferred, not required)
\n
