Adult Case Manager | Certified Mental Health Specialist

Join a dedicated team delivering interactive mental health services to adults and children facing significant mental health challenges in a supportive community setting. The Adult Case Manager plays a vital role in enhancing overall functioning and promoting independence by helping clients remain in the least restrictive environment possible, typically their home and community.

Essential Responsibilities
  • Provide skill training focused on target goals with measurable outcomes, as determined by a Qualified Mental Health Professional.
  • Deliver intensive, community-based mental health services using a strengths-based, skill-building approach.
  • Collaborate with clients, families, and treatment teams to develop and maintain client-centered treatment plans.
  • Help clients understand their symptoms and explore potential solutions.
  • Support clients and families in leveraging natural community supports to foster problem-solving.
  • Coordinate mental and physical healthcare services, facilitating communication among clients, providers, families, and community partners.
  • Adhere to privacy and security regulations concerning Protected Health Information (PHI).
  • Maintain professional and effective communication with clients, team members, and external agencies.
Education and Experience
  • Required: Bachelor's degree in Social Work, Psychology, or related field; or Associate's degree plus 2+ years of relevant experience educating and teaching clients about self-care and daily living activities; or equivalent combination totaling 4+ years.
  • Preferred: Bachelor's degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics.
  • Preferred experience: 3-5 years in mental health services, crisis response, or related fields.
Licenses and Certifications
  • Required: CPST Case Management training, KHS training, background checks including KBI, DCF child abuse, adult abuse registry, and motor vehicle screening.
  • Completion of Adult Case Management training within 60 days of employment.
  • Valid driver's license, insurance, and reliable vehicle if driving is required.
Skills and Abilities
  • Familiarity with community mental health services.
  • Strong organizational skills.
  • Intermediate proficiency with Microsoft Office.

Physical Requirements

Light work involving occasional exertion up to 25 lbs and frequent exertion up to 10 lbs. Standing or walking may be required 11-25% of the day.
