Home Health Licensed Practical Nurse (LPN)

Schedule: Monday–Friday, 8:00 AM–5:00 PM
Evening & Weekend On-Call Rotation Required

We're seeking a compassionate and dependable Home Health Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes throughout North Central West Virginia. Position Details

• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• On-Call: Evening and weekend rotation required
• Productivity Requirement: 32 points per week
• Mileage Reimbursement: $0.54 per mile
• License Required: Active West Virginia Licensed Practical Nurse (LPN) License

Coverage Area

This position serves patients in the following West Virginia counties:

• Harrison
• Marion
• Taylor
• Barbour
• Doddridge
• Lewis
• Upshur

Responsibilities

• Provide skilled nursing care in accordance with each patient's individualized plan of care.
• Monitor and document patient conditions, including vital signs, and promptly report significant changes.
• Administer medications, oxygen therapy, enteral feedings, ostomy care, and other prescribed treatments.
• Collaborate with physicians, Registered Nurses, Physical Therapists, Occupational Therapists, Speech Therapists, and other healthcare professionals to ensure coordinated care.
• Reinforce patient and caregiver education to promote independence and improve health outcomes.
• Maintain accurate and timely clinical documentation using HomeCare HomeBase (HCHB).
• Follow infection prevention and control protocols to ensure a safe care environment.
• Order and monitor medical supplies, medications, and equipment as needed.
• Respond appropriately to patient incidents, complete required documentation, and communicate with the care team.
• Ensure medical equipment is functioning properly and coordinate repairs when necessary.
• Deliver compassionate, high-quality care while preserving each patient's dignity, comfort, and independence.

Required Qualifications

• Active West Virginia Licensed Practical Nurse (LPN) license.
• Home Health experience required.
• HomeCare HomeBase (HCHB) experience required.
• Strong clinical assessment and documentation skills.
• Excellent communication and time management abilities.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work independently while managing a designated patient territory.
