The Alps at Wayne Rehabilitation & Healthcare Center is seeking a compassionate and dedicated Part-Time Social Worker to join our interdisciplinary team. This position offers an opportunity to advocate for residents, support families, and help ensure the highest quality of life for individuals receiving short-term rehabilitation and long-term care services. The ideal social worker candidate is caring, organized, and passionate about helping residents navigate the emotional, social, and practical challenges that can arise during their healthcare journey. Flexible part-time schedule Supportive and collaborative work environment Opportunity to make a meaningful impact on residents and families Professional growth and development opportunities Stable and respected healthcare organization Responsibilities of Social Worker Provide psychosocial support and counseling to residents and their families Complete social service assessments and maintain accurate documentation Assist with care planning and interdisciplinary team meetings Support discharge planning and coordination of community resources Address resident concerns and advocate for resident rights Facilitate communication between residents, families, and healthcare staff Assist with admissions and adjustment-to-facility processes Participate in resident and family meetings as needed Ensure compliance with federal, state, and facility regulations Qualifications for Social Worker Bachelor's Degree in Social Work or related field required Experience in a skilled nursing facility, rehabilitation center, healthcare setting, or long‑term care environment preferred Knowledge of discharge planning, resident rights, and care coordination Strong communication and interpersonal skills Ability to work effectively as part of an interdisciplinary team Experience with electronic medical records preferred At The Alps at Wayne Rehabilitation & Healthcare Center, we are committed to providing exceptional care while creating a supportive environment for both residents and employees.
