Seven Healthcare are seeking a dedicated Physical Therapist for a Travel assignment in Ashland, KY. This role focuses on home health physical therapy, providing comprehensive patient evaluations and treatments based on physician referrals. Work within a supportive and professional environment, covering counties including Boyd, Greenup, Lawrence, Carter, and Elliot.

- Perform physical therapy evaluation and treatment of patients based on physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for current diagnosis, past medical history, precautions, and contraindications
- Establish measurable goals and develop treatment plans to maximize patient independence and function
- Regularly re-assess treatment effectiveness, patient progress, and modify goals or treatments as necessary
- Communicate patient needs and progress to involved parties, including the treatment team and family members
- Consult with medical providers regarding patient evaluation, treatment progress, and discharge
- Recommend and assist in acquiring adaptive equipment to enhance rehabilitation programs
- Maintain and submit documentation according to regulations and company standards
- Adhere to the Company's Code of Business Conduct and professional practice standards
- Supervise Physical Therapist Assistants and Aides
- Participate in staff development programs and maintain clinical practice skills
- Kentucky Physical Therapist Licensure or Compact Licensure
- 1 year of home health physical therapy experience
- Previous OASIS experience
- Familiarity with home care home base
