Description
- Conduct evaluations of patients in inpatient or outpatient settings and create tailor-made treatment plans
- Implement physical therapy interventions centered around mobility, strength, balance, and overall functionality
- Collaborate with nurses, physicians, case managers, and various rehabilitation professionals to enhance patient outcomes
- Guide patients and families in understanding therapeutic goals, discharge procedures, and at-home exercises
- Meticulously document evaluations, treatments, and patient progress in compliance with facility and regulatory practices
- Take an active role in care team discussions and interdisciplinary meetings for optimal care coordination
Education
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
Certifications
- Valid state licensure as a Doctor Of Physical Therapy
- CPR certification is preferred
- Specialized certifications (e.g., neurological, geriatric) are advantageous but not required
Skills
- Prior experience is strongly preferred
- Capability to treat patients presenting with intricate medical and mobility challenges
- Excellent communication and teamwork abilities
- Comfortable utilizing digital charting and clinical documentation systems
