Practice Setting: Outpatient, Inpatient

Does your Facility require physicians to be board certified: Yes

Fellowship: N/A

Minimum Board Certification: Certified

What is the license requirement to bid?: Licensed in WI only

Estimated Credentialing Timeframe (Days): 90 or Act 10

Additional shifts needed as soon as 1-6-2025.

This need is for a mid level or MD provider coverage of clinic out patient WOUND clinic. Clinic is scheduled

Monday to Thursday. Does need the ability to see inpatients in hospital for wound consults. Be able to do outpatient clinic based procedures. No call
