Job Requirements

• Active LPN license in good standing
• At least 1 year of nursing experience required; home health or long-term care experience preferred
• Comfortable traveling within an assigned service area for patient visits
• HCHB experience required
• Strong clinical documentation skills and ability to meet next-day charting expectations
• Ability to manage a productive caseload while delivering high-quality, compassionate care
• Excellent communication, organization, and time-management skills

Responsibilities

• Provide skilled nursing care to patients in a home health setting
• Travel throughout the assigned territory to complete patient visits
• Document care accurately and ensure charts are completed by the following morning
• Maintain productivity standards while delivering efficient, high-quality care
• Support care coordination and communicate updates with the care team as needed
• Promote patient comfort, safety, and continuity of care
• Use HCHB to complete visit documentation and workflow tasks
