Family Health Center in Kalamazoo, MI is seeking a locum or locum to perm General Dentist for ongoing coverage.

Practice Setting: Group Practice FQHC
Facility interested in Locums, Locums to Perm, or Perm candidates!
12-15 ppd
EMR: Dentrix and Epic
Estimated credentialing timeframe: 60-90 days

Requirement description : DDS/DMD - Required
MI state license - Highly Preferred
BLS - Required
ACLS or PALS - Required Clean background/no malpractice - Highly Preferred
Provider must be fully vaccinated for Covid -Required
CERTIFICATION REQUIREMENTS :

ACLS
BLS
PALS
Board Certified

STATE LICENSE REQUIREMENTS :

Michigan

Weekend Requirements : Rotating Saturday, 830a 5p
