With a staff of 950 and an annual budget of $140 million Project Renewal is one of the larger social service nonprofits in New York : Nurse Care ManagerProgram Psychiatric ServicesSchedule: Monday Friday 9:00am - 5:00pmSalary:$120000Program Overview:The Nurse Care Manager Psychiatric Services reports to the Medical Director of Psychiatric Services. Collaborates with interdisciplinary team including Primary Care Drug User Health Nursing PEQA/QA and Healthcare Duties and Responsibilities:The essential duties of theNurse Care Manager Psychiatric Services include but are not limited to the following activities:Clinical Meets with new clients to review and document psychiatric and medical history and assess learning needs and social determinants of health metrics as they relate to psychiatryAttends high risk and interdisciplinary meetings at specific sitesProvides counseling and education for clients about their care plan and healthcare optionsProvides care management support for psychiatric clients including triage of and assistance with psychiatric or pharmacy issues that arise in between appointmentsCollaborates with psychiatry nursing primary care and drug user health programs at the assigned sitesProvides care coordination with EMS hospitals and higher levels of care (ACT IMT SOS Co-response teams)Provides clinical support and training to clinical and non-clinical staffFacilitates referrals to community-based counseling and other community-based resources to address social determinants of patient interactions in the Electronic Health RecordOutreaches clients after incidents in shelter or housing as well as outreach those who may be symptomatic and have difficulty attending apptsSupport clients who may be hospitalized with care coordination and transition back to the communityParticipate in weekly supervisionCoordinate with Program staff to support psychiatric needs of clientsAdminister LAIs as neededSupports new projects as indicated within scope of licenseQuality AssuranceAssists in development of quality assurance measuresAssists and supports Medical Director CQI Project Manager with chart reviews and clinical data reviewWork with QA team to ensure proper documentation in EMR and support any changes to specific workflowsSupport building out new workflows and utilizing proper templates for data collection in the electronic health recordParticipate in meetings with QA department to review metricsThis role will travel as needed to support Psychiatric services and training at all PRI :New York State RN LicenseBLS Certification2-3 years of experience working as a registered in community settings with people experiencing serious mental illness and people experiencing homelessness. Must have excellent oral and written have the ability to work independently prioritize tasks and coordinate schedules including at different have the ability to interface with clients and staff with a broad range of socioeconomic and cultural have knowledge of Microsoft Office especially Word and Excel.
