This position will promote patients potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge./ph3Qualifications/h3pQualifications:/pulliRegistered Nurse, licensed per the state requirements./lili2 years of experience in Home Health preferred./liliHomeCare HomeBase experience preferred./liliGood communication skills, assessment skills, and strong clinical documentation skills./liliAbility to read, write, and speak the English language./liliReliable transportation. Valid and current auto liability insurance./li/ulpCase Manager Essential Functions:/pulliSupervises and manages all aspects of patient care, including LVN and HHA supervision/liliCoordinates the distribution of visits among all disciplines (SN, PT, OT, ST, MSW, HHA) after the Start of Care, Recertification or Resumption of Care is completed, and coordinates POC with the physician. Ensures all visits are provided as ordered./liliInitiates referrals to appropriate ancillary services i.e. podiatry, community services./liliFacilitates interdisciplinary communication to ensure that patients needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress./liliLeads weekly interdisciplinary case conferences for patient caseload, and contributes to the development of care plans for patients./liliEstablishes specific social action plans and realistic goals for each patient consistent with medically related social and emotional needs./liliWith the Administrator, interprets and promotes patients rights and the Patients Bill of Rights./liliCoordinates all aspects of discharge planning with patients, clinical team and agency/liliProvides individual and group help for patients/residents and families at times of adjustment, crisis or particular need./liliCompletes all necessary and supportive documentation-assessments, evaluations, visit notes and discharges, in accordance with state and federal regulations./liliAbility to function as a team leader/role model./liliServes on various committees as requested by Administrator./liliHas reviewed TheraCare Home Health Clinical Policies and Procedures on Abuse Prevention and knows the employees responsibility to enforce it./liliResponsible for assuring patient safety./liliPerforms other duties/tasks as assigned./li/ulpWe are an Equal opportunity employer; We offer an excellent benefit plan to include 401K with match, CEU reimbursement, vacation, sick, holidays, medical, dental, and supplemental insurance Plans as well as a Highly competitive compensation package./p/div
