POSITION SUMMARY - RN Case Manager


POSITION DUTIES - Coordinate and oversee patients' interdisciplinary care plans, assess clinical and psychosocial needs, ensure appropriate resource utilization, facilitates discharge planning, and act as a liaison between patients, families, and the healthcare team to support optimal recovery outcomes.


MINIMUM REQUIRED QUALIFICATIONS -

• AR/compact RN license
• Minimum of two-year nursing degree
• BLS
• 2 years recent acute hospital case management experience

PREFERRED QUALIFICATIONS -

• Acute inpatient rehab experience
• Previous experience in clinical liaison, marketing, or healthcare sales

LENGTH OF ASSIGNMENT - 13 weeks


SHIFT / HOURS PER WEEK - 8a-5p with scheduled weekend coverage


SYSTEMS - Meditech
