• Seeking PMR for expanding Rehab Department.  
• Serve population of over 800,000 within a 42-county area in southwest Georgia
• Opportunity for hybrid position (both in and outpatient) or 100% inpatient.  
• 25 Plus Inpatient Rehab and licensed for 24 LTACH (consult opportunities)
• Support full continuum of post-acute care including LTACH, skilled nursing, hospice, home health, and outpatient rehabilitation
• Hospitalist team medically manages 100% of all admissions
• Designated a Primary Stroke center for its excellence in stroke care and prevention
• Inpatient Rehabilitation Unit fiscal year data:
  • 25+ IPR beds with potential for more
  • Total admissions = 257
  • CMI = 1.376
  • Discharge to community: 79%
  • Functional change: above national average (+3.7 points)
  • Patient Experience: 84%
• Provide care for patients impacted by multi-trauma, other non-traumatic neurologic disorders, cardiac disease, and
  amputation.
