After-Hours Hospice RN Care Manager

Gentiva

Gentiva seeks an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will perform assessments, admissions, and visits during nights, weekends, and on-call, ensuring continuous, coordinated care for patients and families.

Responsibilities include leading IDG communications, updating plans of care, and maintaining accurate clinical documentation while adhering to hospice standards and regulatory requirements.
