Our clinicians travel to patients’ homes to provide continuous care for chronic conditions and limited mobility, with programs in remote patient monitoring, behavioral health management, and chronic care management. Job Summary We are seeking an experienced Hospice Chaplain to work on a per diem basis within the Oklahoma City, OK market. The chaplain will provide pastoral and spiritual care to patients nearing end of life and support families throughout the bereavement process, working under the supervision of the Clinical Director. Responsibilities Provide pastoral/spiritual care to patients and families. Visit and minister with patients, coordinating a spiritual care plan with the interdisciplinary team. Conduct spiritual assessments and develop care plans addressing physical, emotional and spiritual needs. Offer pastoral support at bedside, funerals and memorials as requested. Document visits in line with company and compliance policies. Serve as a member of the interdisciplinary team. Organize company‑sponsored memorial services when appropriate. Offer emotional/spiritual support to staff members. Participate in staff meetings, interdisciplinary team meetings and in‑services. Maintain communication with area inter‑faith clergy. Act as a bridge between patients, families and clergy to provide end‑of‑life sacramental support. Educate staff on specific religious needs and issues affecting patient care. Perform additional duties as assigned or requested. Report primarily to the Clinical Director. Collaborate with the interdisciplinary team to ensure holistic patient care. Contribute value‑added activities that enhance well‑being among clients, patients, families and staff. Education & Qualifications Master of Divinity or equivalent theological degree required. At least four units of Clinical Pastoral Education (CPE). Board Certified as a Chaplain or eligible for Board Certification under ACPE or CPSP. Excellent communication and pastoral skills. Strong emotional energy, mental alertness, and ability to support patients and loved ones through pain, suffering, death and grief. Good organizational and decision‑making skills. Computer literacy and ability to perform extensive charting using the company’s EMR system. Licensure Certification or eligibility for certification through CPSP or ACPE. Experience Minimum two years of pastoral experience in a healthcare setting. Hospice experience desirable. Demonstrated empathy, compassion, sensitivity, and ability to support inter‑faith families. Professionalism / Growth Strong interpersonal skills, working effectively with diverse ages, lifestyles, cultures, socio‑economic and religious backgrounds. Ability to collaborate with physicians, nurses, social workers, clients and other healthcare team members. Independence, initiative, and accountability. Physical and emotional health required.
