Case Manager: Empower Clients toward Self-Sufficiency
Love Your Neighbor Collaborative (LYNC) at Path of Life
Path of Life Ministries (POLM) is hiring Case Managers to work with shelter guests and POLM participants. You will assess assets and needs, develop and monitor Individual Action Plans, and help clients reach self-sufficiency through targeted resources and support. Responsibilities include maintaining client files, documenting progress, and providing weekly reports to program directors. A relevant degree and at least two years in case management are preferred, with strong communication and #J-18808-Ljbffr Love Your Neighbor Collaborative (LYNC) at Path of Life
