Job Summary
Intersect Healthcare of Elmwood is seeking a dedicated and compassionate MDS to join our interdisciplinary clinical team at our 110-bed facility in Detroit, Michigan. As an MDS, you will play a vital role in ensuring the highest quality of care for our residents by collecting and analyzing data to develop individualized care plans. Responsibilities
Collect and analyze data for the Minimum Data Set (MDS) 3.0 assessment
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Ensure accurate and timely completion of MDS assessments and care plans
Utilize clinical knowledge and expertise to identify and address resident needs
Collaborate with the nursing team to implement care plans and achieve optimal resident outcomes
Participate in quality improvement initiatives to enhance resident care and satisfaction
Maintain accurate and detailed records of resident data and care plans
Requirements
Bachelor's Degree in Nursing or related field
Current RN license in the state of Michigan
Minimum of 0 years of experience in a long-term care setting
Strong clinical knowledge and analytical skills
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment and prioritize tasks
Commitment to providing exceptional resident care and customer service
