***$20,000 Sign-On/Retention Bonus available for Qualified Candidates*** As the Registered Nurse (RN), you'll be under the supervision of the Hospice Supervisor, utilizing the nursing process of ongoing assessment, planning, intervention, implementation and evaluation, is responsible for identifying patient/family needs and providing quality care/services in accordance with the attending physician's orders, plan of care and Agency policies and procedures. Responsible for performing patient care on an intermittent basis in the patient's place of residence in close coordination with other disciplines and caregivers involved in the patient's care.

Coverage Area: Clearfield

• Fleet vehicles available for eligible employees

• Fleet Vehicles are permitted to be used for work AND personal use
• All fuel costs for a fleet vehicle (for work AND personal) are covered by PHH

Qualifications: -Be a Registered Nurse with a valid PA Nursing License -Previous home health/long term care experience preferred -Have ability to work well independently and within a multidisciplinary team -Have valid PA driver's license, current auto coverage and current auto coverage
