LPN/RN (Nights)

Overview $1,500 sign‑on bonus. LPN: Fri, Sat 10p‑7a (shift differential for 11p‑7a). Homecare with Heart offers the most competitive pay scale. We are looking for compassionate and experienced nurses to provide care for our clients in their homes. Starting rate for LPN: $26 per hour, RN: $30 per hour depending on experience and availability. Responsibilities Administer medication in accordance with standards of practice. Encourage social and rehabilitative skills and encourage self‑care and independence. Perform client treatments in accordance with Plan of Care. Assist the consumer in learning appropriate self‑care techniques. Prepare clinical and progress notes to document care provided. Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor. Promote activities to achieve a safe and healthy environment. Observe aseptic technique when performing client procedures/care. Collaborate with other members of the clinical team to develop and improve the Plan of Care. Assist the client in achieving established goals in the Plan of Care developed by the RN. Maintain productivity according to the standards of the agency. Maintain license, CEUs and mandatory employment requirements. Participate in client care meetings, in‑services, orientation and staff meetings. Provide service in accordance with agency’s philosophy, standards, policies and scope of practice. Participate in Performance Improvement and adhere to HIPAA guidelines. Other duties as assigned. Qualifications Join us and be part of our team! Valid State LPN/RN License. Valid driver’s license and automotive use required due to travel involved. Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities. Ability to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures. Ability to use a computer, laptop or tablet. Ability to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
