Specific Duties Include, But Are Not Limited To:

• Accountable for all elements of the nursing process (assessment, diagnosis, outcome identification, planning, implementation, evaluation) when providing and/or supervising direct patient care.
• Coordinates comprehensive care for veterans, with a focus on patient education, self-management, and customer satisfaction throughout the continuum of care.
• Streamlines referral processes by receiving, triaging, and directing timely, coordinated care utilizing a national standardized triage tool in accordance with VHA standards and guidelines.
• Applies clinical knowledge of internal referral care processes, review criteria, utilization management standards, and community care regulations to ensure appropriate care throughout the consult process.
• Serves as a liaison between internal and external providers, effectively guiding veterans through a full range of care options within VA and the community.
• Supports timely access to care, empowering veterans to make informed decisions and ensuring eligible veterans are referred and scheduled appropriately.
• Triages incoming referrals, reviews available care options, provides scheduling guidance, and assists veterans and providers throughout the consult process.
• Collaborates with the office of integrated veteran care to triage requests for services (RFS), promoting in-house access and optimizing utilization of VA resources.
• Contributes professional nursing perspective and clinical expertise to interdisciplinary teams, actively participating in quality improvement initiatives to enhance patient outcomes.
• Leads integration of evidence-based practice and research, assessing and evaluating patient outcomes for continuous improvement in quality of care and resource utilization.
• Utilizes delegation of authority, as appropriate, to function at the highest level of role performance; aligns the delivery of high-quality care with key performance indicators to ensure business practice standards consistently meet patient care needs.

Pay:

Competitive salary, regular salary increases, potential for performance awards
