Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois Physical Therapist license required to start the assignment.
• BLS (Basic Life Support) certification from AHA or ARC may be required.
• Experience working in skilled nursing or long-term care settings preferred.
• Ability to manage caseloads typical of post-acute and skilled nursing populations, including restorative and skilled therapy interventions.

Preferred Qualifications

• Previous travel therapy experience preferred.
• Experience with electronic medical records preferred.

Assignment Details

• Shift hours: 08:30 - 16:30.
• Work pattern: 5 shifts per week, 8 hours per shift.
• Guaranteed hours: 35 hours per week.

Impacting Patient Care

Provide evidence-based physical therapy to skilled nursing residents to improve mobility, function, and quality of life. Collaborate with interdisciplinary teams to develop and implement individualized care plans. Document outcomes and progress in the medical record to support continuity of care and regulatory compliance.
