Position: Director of Social Services

Location: Palm Beach County, Florida
Schedule: Full-Time | 8:00 a.m. – 5:00 p.m.
Salary: $75,000–$78,000 per year

Healthcare & Rehabilitation is seeking an experienced and compassionate Director of Social Services to lead our social services department and advocate for the well-being of our residents and their families. • Develop, implement, and maintain departmental policies in compliance with federal and state regulations

• Lead, mentor, and support the Social Services team

• Advocate for resident rights and promote a culture of dignity, compassion, and respect

• Participate in interdisciplinary care planning meetings

• Coordinate and oversee discharge planning to ensure smooth resident transitions

• Monitor and enhance the quality of social services to improve resident outcomes

• Ensure accurate documentation and regulatory compliance

• Prepare and present Quality Assurance (QA) reports

• Serve as the facility’s Grievance Officer, addressing concerns with professionalism and empathy

• Bachelor’s Degree in Social Work (BSW) or related human services field preferred

• Minimum of 2 years of experience in a healthcare or long-term care setting

• Strong knowledge of federal and state long-term care regulations

• Excellent communication, leadership, and organizational skills
