Job Summary

As an LPN MDS Coordinator at Green Palms Health & Rehab of St. Marys, you will play a vital role in ensuring the high-quality care and comfort of our residents. Responsibilities

• Coordinate the completion of the Minimum Data Set (MDS) assessments for our residents, ensuring compliance with regulatory requirements and company standards

• Collaborate with the healthcare team to develop and implement individualized care plans that address the unique needs and goals of each resident

• Monitor and document changes in resident condition, including physical, cognitive, and emotional status

• Communicate with residents, families, and healthcare providers to ensure accurate and timely information exchange

• Utilize your strong organizational skills to manage and prioritize tasks, ensuring seamless coordination of care

• Maintain accurate and up-to-date records, adhering to HIPAA and regulatory guidelines

• Participate in quality improvement initiatives, contributing to the growth and development of our care team

Requirements

• Licensed Practical Nurse (LPN) license in the state of Ohio

• High school diploma or equivalent required; ADN or RN degree preferred

• 0-2 years of experience in long-term care or similar setting, with a focus on MDS coordination

• Strong communication, organizational, and problem-solving skills

• Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
