We‘re looking for a full-time Primary Care Nurse Practitioner who excels in a collaborative, patient-centered environment and is committed to improving outcomes for adult and geriatric patients.

Responsibilities:

• You will report to the Associate Medical Director
• You will work fully onsite in Homeland Park, SC
• Provide comprehensive outpatient care for senior patients
• Maintain accurate documentation and medical records
• Diagnose and manage moderately complex conditions
• Coordinate referrals and collaborate with specialists
• Partner with interdisciplinary teams for holistic care

Use your skills to make an impact

Required Qualification:

• Graduate of an accredited Nurse Practitioner program (MSN or DNP)
• Current national certification (AANP or ANCC)
• Active, unrestricted NP license in SC.
• DEA license and Medicare/Medicaid credentials (or ability to obtain upon hire)

Preferred Qualification:

• 2+ years Nurse Practitioner experience
• Experience in primary care, or Medicare Advantage settings with geriatric patients
• Experience with Athena EMR
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Work Environment

• Compensation package including sign-on bonus, relocation assistance, and comprehensive benefits.
• Outpatient clinical setting.
• Standard schedule: Monday–Friday, 8:00 AM – 5:00 PM

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $123,800 - $170,300 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
