Candidates should be prepared to conduct both telehealth and in-field assessments. Nurses will perform initial in-home pre-evaluations of patients. These evaluations include assessing health status, strengths, care needs, and preferences, guiding the development of individualized long-term care service plans. Candidates will review consumer medical documentation and health referral forms relevant to each case. Documentation of any concerns or conflicting information is essential during the evaluation process. Nurses will emphasize continuity of care to reduce fragmentation and gaps in treatment plans. Communication with consumers about healthcare options and necessary supports is critical. Candidates will initiate communication for enrollment into managed care health plans. Enhancing communication and collaborative relationships with interdisciplinary care team members is a vital responsibility. Collecting data for quality reviews and documenting case notes is required. Nurses will identify opportunities for health promotion and illness prevention. Maintaining knowledge of community resources, payor requirements, and network services for the target population is necessary. All candidates must meet the established performance standards for this position. Additional duties may be assigned as requested by management.
