In general, our agency provides services Mon‑Fri from 8:00am to 5:00pm. Salary Range: $28.13 to $36.37 (hourly). There are 15 steps on our salary scale. We use a bona fide factor system to determine starting wage (years of experience, education, credentials, and specialty skills that relate to the job requirements). Combination of education/experience that demonstrates the ability to perform the duties outlined for this position may be considered. Summary: The Care Coordinator is responsible for coordinating/supporting behavioral health care within CCMH and the referrals to clinically indicated services and supports outside of the agency. Essential duties may vary per location/department/position. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required: Experience working with underserved, transient populations; experience working with consumers who have co‑occurring mental health substance abuse and physical health issues preferred. Personal experience assisting self or family member in recovery from a behavioral health condition helpful. Be able to collaborate effectively in a team setting, maintain effective and professional relationships with consumers and members of the Care Team; strong written and verbal communication skills. Must have the ability to effectively engage consumers in a therapeutic relationship when appropriate; and able to navigate the pace of a community behavioral health care setting. Problem solving skills, communication with other professionals, time management, note taking, and ability to remain calm in high stress situations a must. Mental Health case management skills preferred. Must have reliable transportation.
