Patient Care Coordinator/RN Case Manager
Position Duties
- Strong discharge planning and care coordination experience in an inpatient hospital setting.
- Kaiser Permanente experience, particularly in utilization review and discharge planning workflows.
- Familiarity with post-acute placement processes, including SNF, home health, DME, transportation, and community resource coordination.
- Experience collaborating with multidisciplinary teams, physicians, nursing, social work, and external agencies to facilitate timely and safe discharges.
Minimum Required Qualifications CA license; must have Kaiser experience
Length Of Assignment 13 weeks
Shift / Hours Per Week Schedule: 8:00 AM to 4:30 PM
Week 1: Mon, Tue, Thur, Fri, Sat
Week 2: Sun, Mon, Wed, Thu, Fri

- Strong Epic documentation and communication skills.
