About the Role: The Case Manager for the Early Intervention Psychosis Program plays a critical role in providing comprehensive support and coordination of care for individuals experiencing early signs of psychosis. This position focuses on facilitating timely access to mental health services, promoting recovery, and improving overall quality of life for clients through personalized care plans. The Case Manager collaborates closely with multidisciplinary teams, including psychiatrists, therapists, and social workers, to ensure integrated and holistic treatment approaches. They serve as a primary point of contact for clients and their families, offering education, advocacy, and resource linkage to community supports. Ultimately, this role aims to reduce the impact of psychosis by fostering early engagement, continuous monitoring, and sustained recovery efforts within the community setting. Minimum Qualifications: Bachelor’s degree in Social Work, Psychology, Nursing, or a related human services field. At least two years of experience working with individuals experiencing serious mental illness, preferably psychosis. Knowledge of mental health systems, community resources, and early intervention strategies for psychosis. Strong assessment, case management, and care coordination skills. Ability to maintain confidentiality and adhere to ethical standards in mental health care. Valid Georgia driver's license and valid insurance. Ability to travel frequently in the Metropolitan Atlanta area and access to reliable transportation. Preferred Qualifications: Master’s degree in Social Work, Counseling, Psychology, or related field. Experience specifically in early intervention programs for psychosis or first-episode psychosis. Familiarity with evidence-based practices such as Coordinated Specialty Care (CSC). Certification or licensure relevant to mental health or case management (e.g., LCSW, LPC, RN). Bilingual abilities or experience working with diverse cultural populations. Responsibilities: Conduct comprehensive assessments to identify client needs, strengths, and goals related to early psychosis intervention. Develop, implement, and regularly update individualized care plans in collaboration with clients, families, and clinical teams. Coordinate and facilitate access to psychiatric, therapeutic, social, and community resources to support client recovery. Monitor client progress through regular follow-ups, adjusting care plans as necessary to address evolving needs. Provide education and support to clients and their families about psychosis, treatment options, and coping strategies. Advocate on behalf of clients to ensure they receive appropriate services and support within healthcare and community systems. Maintain accurate and timely documentation of client interactions, care plans, and outcomes in compliance with program standards. Participate in multidisciplinary team meetings to discuss client progress and contribute to treatment planning. Assist clients in developing life skills, including medication management, socialization, and vocational training. Respond promptly to crises or urgent client needs, coordinating emergency interventions when necessary. Skills: The Case Manager utilizes strong interpersonal and communication skills daily to build trusting relationships with clients and their families, ensuring engagement and adherence to treatment plans. Critical thinking and problem-solving skills are essential for assessing client needs, developing effective care strategies, and adapting interventions as client circumstances change. Organizational skills support managing multiple cases simultaneously, coordinating services, and maintaining detailed documentation to meet program requirements. Collaboration skills enable effective teamwork with clinical providers and community partners to deliver integrated care.
