RN Registered Nurse IMCU PT Nights

Overview Manages individualized, outcome oriented nursing care through the use of the nursing process. Provides care, treatment and services through the successful coordination and completion of a series of processes that include assessment, planning care, providing care, in accordance with departmental and hospital policies and procedures. Location and Details Requisition Number : R-22286 Facility : LOC0001 - 55 Fogg Road, 55 Fogg Road, Weymouth, MA 02190 Department : SSH IMCU Pratt 6 South Status : Part time Budgeted Hours : 24 Shift : Night (United States of America) Responsibilities Manages individualized, outcome oriented nursing care through the nursing process. Provides care, treatment and services through coordination and completion of processes including assessment, planning care and providing care in accordance with policies and procedures. Medication Administration: Demonstrates knowledge of and follows SSH policies and procedures for administering, transcribing, and recording medications. Completes medication reconciliation process following SSH policy and procedure. Demonstrates proper procedure for documentation of narcotic withdrawal, administration, verifies count, wastes per policy and resolves narcotic discrepancy. Plan of care/Documentation/Patient Family Centered Care/Patient Experience: Develops, discusses, and communicates a prioritized problem list/plan of care for each patient. Develop, evaluate and update individualized plan of care for patient and documents outcomes. Initiating admission assessment within 8 hours of admissions; identifies and documents patient/family/significant other teaching needs upon admission and throughout hospitalization. Continues assessment/reassessment and identifies care needs within established nursing practice. Documents all patient care following the department of nursing policy, unit based standards, disease processes and nurse sensitive indicators (falls, skin, CAUTI). Assess/reassesses and documents patient's level of comfort utilizing the appropriate pain scale and the patient's response to each intervention both pharmacological and non-pharmacological per policy. Works on behalf of patient/family; seeks help to represent patient/family when they are unable to represent themselves. Raises ethical questions and concerns with clinical team; seeks available resources to help formulate and understand ethical decisions. Actively communicates plan of day via white board updating every shift. Facilitates discharge/transfer by goal of 12:00pm; communicates barriers utilizing chain of command. Safety/Quality: Fosters a "Culture of Safety" through personal ownership and commitment to a safe environment. Verifies patient identification with two identifiers prior to the start of any invasive procedure, including time out, administration of care, medications, and documentation confirming the correct patient, procedure, site, equipment and consent. Complies with hand hygiene guidelines, adheres to universal precautions, uses PPE as required, disposes of hazardous materials properly, and maintains awareness of MSDS sheets. Ensures environment meets regulatory requirements at all times. Understands and demonstrates roles in hospital codes/emergency preparedness. Professional Development: Assumes responsibility for own professional development by incorporating evidence-based practice, research, and performance improvement initiatives. Obtains at least 5 contact hours per year in area of practice. Practices within the MA Nurse Practice Act; directs other licenses and non-licensed personnel as assigned. Displays the ability to accept and respond to feedback and recommendations for change. Demonstrates professional working relationships with colleagues from all disciplines to promote a positive workplace. Critical Thinking: analyzes causes of problems, evaluates solutions, and communicates effectively via chain of command. Keeps informed about changes in hospital policy and procedure. Technology: Uses technological solutions to support work processes and practices; accesses Lotus Notes for email, LMS and other RN resources; utilizes department and unit software applications; understands downtime operations. Compliance: Works within legal, regulatory and ethical standards; protects patient privacy and security; reports potential compliance issues to management; completes mandatory education for compliance, privacy and security programs. Patient and Family Centered Care: Respects patient values, preferences and expressed needs; collaborates with patient and family in planning, implementing and evaluating care; welcomes family participation per patient preferences. Age & Culture: Demonstrates knowledge and awareness of age and cultural considerations; interacts with diverse populations with sensitivity; utilizes resources such as interpreters as needed. Qualifications Minimum Education - Preferred : Graduate of an accredited School of Nursing Minimum Work Experience : Previous Acute Care RN experience preferred Required Licenses/Certifications : RN - Registered Nurse Depending on department, Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), Emergency Response Training, Neonatal Resuscitation Program (NRP), Pediatric Advanced Life Support (PALS), Trauma Nurse Core Course (TNCC), Trauma Care After Resuscitation (TCAR) may be required Schedule 7p-7:30am rotating weekends and holidays #J-18808-Ljbffr South Shore Health
