Minimum Requirements

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Washington State PT license required to start the assignment.
BLS certification from AHA or ARC may be required.
Minimum one year of home health physical therapy experience required.
Proficiency with HCHB (Homecare Homebase) required.
Ability to travel and provide care within a 50+ mile radius of the assigned office covering Lake Stevens and Snohomish areas.
Productivity expectation: minimum 30 points per week. Documentation must be completed by 10:00 AM the following day.
