Job Summary:
Brown's Health and Rehab is seeking a skilled and compassionate Wound Care RN to join our clinical team. The Wound Care RN will be responsible for assessing, planning, implementing, and evaluating wound care treatment plans for residents in collaboration with the interdisciplinary team. This position plays a vital role in ensuring optimal healing outcomes, infection control, and the highest standard of resident care.

Key Responsibilities:

• Provide comprehensive wound assessments and develop individualized treatment plans.

• Administer appropriate wound care treatments, including dressing changes, debridement, and wound vac management.

• Monitor healing progress, document outcomes, and update care plans accordingly.

• Educate staff on best practices in wound prevention, identification, and care.

• Collaborate with physicians, dietitians, therapists, and other members of the healthcare team to coordinate holistic wound management.

• Maintain accurate and timely documentation in the electronic medical record (EMR) system.

• Ensure compliance with federal, state, and local regulations as well as facility policies and procedures.

• Participate in wound rounds and quality assurance/performance improvement (QAPI) initiatives.

• Provide training and guidance to CNAs and LPNs regarding skin integrity protocols and pressure ulcer prevention.

Qualifications:

• Current and valid RN license in the state of Georgia.

• Wound care certification (WCC, CWCN, or equivalent) preferred or willingness to obtain.

• Minimum of 1–2 years of nursing experience in long-term care or rehabilitation setting; wound care experience strongly preferred.

• Strong clinical assessment skills and attention to detail.

• Excellent communication and documentation skills.

• Ability to work effectively both independently and as part of a team.
