POSITION SUMMARY – RN Case Manager
POSITION DUTIES
Collaborates with providers and interdisciplinary team members to coordinate care. Facilitates referrals, follow-up care, and continuity of services. Documents assessments, interventions, and outcomes accurately and timely in the electronic health record. Advocates for patient care and assists patients in navigating the healthcare system.MINIMUM REQUIRED QUALIFICATIONS
Minimum 1 year of case management experience required within the last 2 years in a clinical setting (hospital or outpatient or ambulatory). Current full, unrestricted Registered Nursing license in Alaska or other U.S. state required. Agency staff must have an active Alaska license. Graduate of an accredited school of nursing. Basic Life Support certification. High school diploma or equivalent required.PREFERRED QUALIFICATIONS
Bachelor’s degree preferred. Case Management certification preferred.LENGTH OF ASSIGNMENT
