Registered Nurse – Home Health travel assignment in Denver, Colorado. Start date 09/08/2026 for 6 weeks, with guaranteed 40 hours per week on an 8-hour shift. Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings and coordinate with the care team.
- Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Obtain data on physical, psychological, social, spiritual, and cultural factors influencing health status; incorporate data into the POC.
- Maintain up-to-date patient records with visits, contacts, problems, plans, actions, and goals; ensure changes are reflected promptly.
- Provide care with proper infection control measures; educate patients and families on care, medications, safety, and self-care practices.
- Supervise and provide clinical directions to HHA/LPN/LVN staff to ensure quality and continuity of service; assist with pain assessment and management; coordinate referrals as needed.
- Participate in on-call responsibilities and serve as liaison to patients, families, team members, and physicians; attend interdisciplinary team meetings as required.

Requirements
- Current Colorado RN license; CPR certification required
- 1–2 years RN experience in a clinical care setting; 2+ years home health experience preferred
- ADN or Nursing Diploma required; BSN (Bachelor of Science in Nursing) preferred
- CHPN certification preferred; knowledge of venipuncture and IV care required
- OASIS experience and Homecare Homebase software experience required
- Must have a valid driver's license and reliable transportation; ability to travel locally
- Able to work in home health/hospice settings; strong communication and organizational skills
