Primary Care Nurse Practitioner (NP)

Seeking an experienced Family Nurse Practitioner to join a patient-centered outpatient primary care team in a respected clinic. Responsibilities include providing continuity of care, chronic disease management (such as diabetes, hypertension, hyperlipidemia), and working with a veteran population. Role offers independent practice with own patient panel within a team-based PACT model supported by RNs, LPNs, MAs, mental health, and pharmacy teams. Key duties involve managing geriatric and multi-morbidity cases in a primarily outpatient setting using CPRS EMR. Qualifications include a Family Nurse Practitioner certification, minimum of 2 years outpatient experience, and comfort practicing independently. Preferred Experience In Veteran Care Is a Plus.
