Patient Care Specialist II

To support the Home Health team, the full-time Patient Care Specialist II will coordinate front office operations, manage patient scheduling, and facilitate communication between patients and care teams in a remote setting. Key Responsibilities Coordinate patient scheduling, registration, visit preparation, and follow-up activities across assigned care center workflows Verify patient demographics, insurance information, and required documentation prior to or during visits Respond to patient inquiries and maintain accurate records in electronic medical record and scheduling systems Required Qualifications High school diploma or GED required 2+ years of progressive experience in a healthcare or patient access environment Strong working knowledge of EHR systems, insurance verification, and medical terminology Demonstrated professionalism and commitment to patient confidentiality Ability to work independently in a fast-paced, cross-functional environment
