6AM to 2:30PM

1PM to 9:30PM

Responsibilities

• Provide direct patient care under supervision, following clinical protocols.

• Accurately document patient assessments and care plans in electronic health records.

• Monitor patient vital signs and report changes promptly to the care team.

• Adhere to regulatory and facility policies, including privacy and confidentiality requirements.

• Assist in routine procedures and administer injections per the physician orders.

• Maintain sterile technique and uphold infection control standards at all times.

• Identify opportunities for professional development and pursue relevant certifications.

• Assist with triage and prioritization to meet targets and ensure timely patient care.

• Collaborate with the healthcare team to optimize patient outcomes within a results-driven environment.

• Educate patients and families on care plans, procedures, and post-care instructions.

• Utilize pipelines and performance metrics to forecast workload and improve service delivery.

Experience/Qualifications

• LPN license valid in the applicable state with current CPR certification.

• Proficient in electronic health records documentation and accuracy in care plans.

• Excellent clinical skills for direct patient care and injections per orders.

• Ability to monitor vital signs, identify changes, and escalate timely.

• Strong infection control practices and sterile technique adherence.

• Experience with triage, prioritization, and meeting patient flow targets.

• Demonstrated commitment to professional development and pursuing relevant certifications.

• Strong teamwork with accountability for individual performance and outcomes.

• Clear patient and family education skills on care plans and post-care instructions.

• Data-driven mindset with ability to use performance metrics to forecast workload.
