POSITION SUMMARY RN Case Manager
POSITION DUTIES Supports safe and timely patient discharge transition (typically 14-18 patients). Coordinates DME and Post acute services. Identifies high-risk patients and discharge barriers, collaborate with interdisciplinary team members, documents and coordinates care in Epic
MINIMUM REQUIRED QUALIFICATIONS
- NYS RN license
- NYS PRI Certification
- Screen Certification
- BLS
- 2 years recent case management experience in an acute hospital setting
PREFERRED QUALIFICATIONS Experience with Epic and Care Port transition
LENGTH OF ASSIGNMENT 8 weeks
SHIFT / HOURS PER WEEK 8:00am-4:00pm, 37.5hrs/week
