Hospice Certified Nursing Assistant (CNA)

Location: Santa Fe and Espanola, NM (Primary Territory) and surrounding area
Home Office: Santa Fe, NM
Position Type: PRN Work as needed

We are seeking a Hospice Certified Nursing Assistant (CNA) to serve patients primarily in Santa Fe and Espanola , providing care in Skilled Nursing Facilities (SNFs), Assisted Livings and patients’ homes . This role requires compassionate hands-on care, excellent communication, and a heart for hospice.

• Provide personal care and emotional support to hospice patients in SNFs and private residences

• Assist with activities of daily living (bathing, grooming, toileting, transfers, feeding)

• Observe and report changes in patient condition to the clinical team

• Support families with education and comfort measures

• Maintain accurate documentation per hospice and regulatory guidelines

• Work collaboratively with nurses, social workers, chaplains, and the interdisciplinary team

• Maintain patient dignity, privacy, and comfort at all times

• Travel throughout the service area daily

• Current New Mexico CNA license (required)

• Hospice or home health experience preferred

• Valid driver’s license, reliable transportation, and auto insurance

• Ability to travel daily within service area

• Strong communication skills and compassionate bedside manner

• Ability to work independently and as part of a team

• Must pass background check and drug screening
