Work Environment

This position is primarily onsite in the homes of our patients, allowing you to deliver care in the most comfortable environment possible for them. The role involves direct patient interaction and assessment, providing you with varied scenarios daily, and the freedom to create meaningful relationships with your patients. Key Responsibilities

• Conduct thorough physical therapy evaluations to assess each patient's strengths and areas for improvement, formulating tailored treatment plans based on individual needs.
• Deliver personalized therapy interventions such as exercise therapy, manual therapy techniques, and functional training to enhance recovery and mobility.
• Educate patients and their families on effective exercise techniques, safety tips, and self-management strategies to empower them in their recovery journey.
• Maintain precise documentation of patient progress, including updates on conditions and treatment adjustments, adhering to regulatory standards and best practices.
• Foster communication and coordinate care with a multidisciplinary team of healthcare professionals, including physicians and nurses, to enhance treatment effectiveness.
• Uphold compliance with all organizational policies, infection control protocols, and safety guidelines to ensure a safe environment for both staff and patients.
• Participate in ongoing professional development and team meetings to stay updated on the latest trends in physical therapy practices.

Patient Population / Client Focus

In this role, you will work primarily with geriatric patients and individuals recovering from surgeries, injuries, or chronic conditions. Your expertise will help these patients navigate their rehabilitation journeys in the comfort of their own homes, enhancing their quality of life and independence. Each day will bring new challenges as you adapt your treatment strategies to meet the diverse needs of your patient demographic.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A current and valid physical therapist license issued by the state of California.
• Minimum of 2 years of experience in a clinical setting, preferably in home care or outpatient physical therapy.

Preferred Experience

• Experience working with elderly populations and those recovering from a variety of medical conditions.
• Familiarity with electronic health record (EHR) systems for documentation and care coordination.
• Strong interpersonal and communication skills to establish rapport with patients and their families.

Compensation and Benefits

The compensation for this full-time position ranges from $110,000 to $125,000 per year, commensurate with experience.
