There is no "one way" to achieve anything and we strive to ensure our team members have the freedom to construct a program that truly puts our patients at the center while also dedicating energy to ensuring each team member feels valued. You will "own" your cases and be encouraged to bring new ideas to support your patients and the departments success. We staff 2+ rehab techs that support you on the floor to make sure you have what you need to provide amazing care.Essential FunctionsEstablishes and administers a treatment plan with specific therapy goals determined according to the patient's capacity and tolerance under the direction of the physician.Assesses patient needs; plans for, evaluates and modifies care to meet goals of physical therapy interventions.Collaborates with all disciplines to plan and evaluate team goals for each patient.Sets realistic and achievable goals for patients/residents.Documents and records the patients'/residents' condition and progress per state and federal regulations.Educates patients and families in an appropriate therapy method.Optimizes the functionality and skills of patients by providing, directing and/or overseeing the screening of all residents.Communicates patient status and needs to the patient, family, and other involved professionals.Ensures punctuality and regular attendance for assigned shifts.Other FunctionsAdhere to Minimum Data Set (MDS) procedures and effective care planning.Provides consultation and clinical supervision to PTA's working in the facility.Other duties as assigned.Knowledge/Skills/AbilitiesAbility to maintain confidentiality.Knowledge of HIPAA.General knowledge and understanding of managed care.Knowledge of state and federal rules relating to therapy delivery and documentation in skilled nursing facilities.RequirementsQualificationsEducation:Bachelor's, Master's or Doctorate Degree from a college or university with an accredited Physical Therapy program.Licenses/Certification:Current Physical Therapist license in the state employed.Current CPR certification.Level 1 Fingerprint Clearance CardExperience:Six months experience in a long-term care environment preferred.Note: All employees of Cascadia Healthcare are required to submit and be cleared to work in the facility per Arizona's specific background check requirements prior to contact with patients/residents.
