Private Home Care is seeking a compassionate and organized Licensed Practical Nurse (LPN) to join our team in a full-time role supporting one of our valued partner facilities. This position is ideal for an LPN who enjoys building relationships, leading caregivers, and making a meaningful impact on both clients and staff.

• Oversee and support Private Home Care caregivers within the partner facility
• Create and manage caregiver schedules
• Build and foster strong client and family relationships
• Complete new client assessments and reassessments
• Develop and update individualized care plans
• Collaborate with facility staff and the PHC team to ensure exceptional care and communication
• Provide ongoing guidance and support to caregivers to maintain quality standards
• Other duties as assigned

• Current LPN license in good standing
• 3+ years of experience in professional setting
• Strong communication and organizational skills
• Experience in home care, senior care, or long-term care preferred
• Leadership experience is a plus
• Passion for providing compassionate, client-centered care
