Job Overview TLC Nursing Associates, Inc. is seeking a Medical Social Worker for travel assignments in hospitals, rehabilitation centers, and long‑term care facilities. The role supports patients and families through emotional support, resource navigation, and care coordination to enhance well‑being and recovery. Responsibilities Conduct psychosocial assessments to identify patients’ emotional, social, and financial needs. Develop and implement care plans addressing patient concerns. Provide counseling and crisis intervention for patients and families. Advocate for patient rights and assist with healthcare decision‑making. Coordinate discharge planning and transition of care services. Educate patients and families about community resources and support programs. Maintain detailed case records and ensure compliance with healthcare regulations. Collaborate with healthcare providers to ensure holistic patient care. Qualifications Master’s degree in Social Work (MSW). Licensed Clinical Social Worker (LCSW) or equivalent state licensure. Additional certifications in healthcare social work preferred. 1+ years of experience in medical or healthcare social work. Strong advocacy and problem‑solving abilities. Excellent communication and counseling skills. Knowledge of healthcare systems and patient rights. Willingness to relocate temporarily for assignments.
