Health Care Nurse

As an integral member of the Yale New Haven Health System (YNHHS) healthcare team, the Registered Professional Nurse (RN) upholds the YNHHS mission, vision, values and strategic initiatives to provide the highest level of patient‑centered care. The RN practices professional nursing as a registered nurse under the legal and ethical framework established by the Connecticut Nurse Practice Act, American Nurses Association Scope and Standards for Nursing Practice, ANA Code of Ethics for Nurses and the YNHHS Professional Practice Model. The RN’s role is exemplified by leadership, delegation, collaboration, effective communication, and a commitment to quality outcomes. Responsibilities Establish and maintain a therapeutic relationship with patients and families, integrating cultural and diversity considerations into care. Communicate and collaborate with patients, families and interdisciplinary health‑care team members. Respect patients’ values, preferences, expressed needs and knowledge during holistic data collection, outcome formulation and evaluation. Apply the nursing process: assessment, diagnosis, outcome identification, planning, implementation, and evaluation. Coordinate care delivery and transitions. Provide health teaching and health promotion strategies to promote health and a safe environment. Evaluate progress toward attainment of outcomes. Participate in professional governance, evidence‑based practice, quality improvement and environmental health initiatives. Qualifications Active Registered Nurse license in the State of Connecticut. Graduation from an accredited nursing school. Demonstrated competence in assessment, diagnosis, planning, implementation, and evaluation of nursing care. Ability to practice ethically and culturally congruent practice. Effective communication, collaboration, leadership, and evidence‑based practice skills. Unit(s) Overview The RN will work in the HVC Cardiac Medical Intermediate Care Unit (5‑2 SP), the HVC Cardiac Surgery Intermediate Care and Step‑Down Unit (5‑3 SP), and the HVC Cardiac Intermediate Care and Step‑Down Unit (V3E). These units provide specialty care for cardiovascular medicine, cardiac surgery, and advanced heart failure and stroke populations, with rapid telemetry monitoring and critical‑care interventions. Additional Information Registered Nursing practice is overseen by the Chief Nursing Officer (CNO) in each delivery network, and RNs are accountable to their chain of command. Nursing practice is guided by the ANA Scope and Standards of Practice (2015) and the Connecticut Nurse Practice Act.
