Compassionate Hospice RN Case Manager

Saint Mary’s Hospice of Northern Nevada, part of the LHC Group and Optum family, is hiring a Registered Nurse Case Manager to oversee hospice care delivery and develop the patient Plan of Care for each patient. You will evaluate the patient’s needs, initiate and coordinate the plan of care with the IDG, document progress and outcomes, and support families through end-of-life care with compassion and professional integrity. #J-18808-Ljbffr
